To prepare for your appointment, please note the following:
Patients
- Arrive 15 minutes early to complete your paperwork
- Bring a photo ID – Driver’s License, State ID, or Passport
Employers
- What type of exam/services are needed?
- If a drug screen is needed, what type and panel?
- Employees under 18 will need to be accompanied by a parent or bring a signed parental consent form.
- Please make sure your employees know what they need to bring to the appointment.
Walk-In Drug Screens
- Patient must have a photo ID
- We accept walk-ins from 7am to 4pm Monday-Friday. We cannot check in drug screens after 4pm.
